There are three forms that you will be kindly requested to complete prior to your first visit.

The first is a health history form outlining your general medical health.  We are currently transitioning to an Electronic Health Record. If your visit with Dr. Rao is scheduled on or after February 15, 2011, you will receive an email with instructions on completing this form online and do not need to download this form here. You may access the portal to securely submit your health information online here. Please keep in mind that you will need a user name and password provided by our office prior to completing this information online. If your visit is prior to February 15, you may download the form here. Please complete this form to the best of your ability.  This will help your doctor & save you time on the day of your visit.   To download a health history form, click here.

The second is a new patient information sheet.  You are not yet able to complete this and the third form online. To download a new patient information sheet, click here.
